Coaching Services
Grounded Path Health provides health and wellness coaching, education, and health-advocacy support. These services are not psychotherapy, medical care, diagnosis, treatment, medication therapy management, or crisis intervention.
Although Samantha Hacker holds PharmD and LMHC credentials, those credentials do not convert the coaching services offered through Grounded Path Health into pharmacy services, medical treatment, or psychotherapy.
Using this website, submitting an inquiry, or communicating with Grounded Path Health does not create a coaching, therapist-client, pharmacist-patient, or other professional relationship. A coaching relationship begins only after Grounded Path Health accepts you as a client and both parties complete the applicable agreement and payment requirements.
